Bridging the Gap Reentry Team - 602-685-6037
The Intern for Bridging the Gap – Re-Entry program shadows Behavioral Health Technicians providing direct care services: intakes, assessments, and coordinates linkages into necessary community services and connections to providers. The Intern helps to coordinate member referrals, learns, and provides case management services as needed, completes tasks that require serving the members who are enrolled in the Bridging the Gap Re-Entry/TIP Justice program; liaises with other Program staff, Program Supervisor, Director and medical records staff. The Intern learns and delivers health and wellness and recovery-oriented services to members. The INTERN participates in vital program evaluation procedures and is responsible for accurate and professional record keeping in required database platforms. This position reports to the Program Manager.
• Support for individuals reentering from the Justice systems.
• Connections to Counseling and Substance Abuse Treatment
• Assistance connecting to medical services/Primary Care Physician
• Resources finding placement at recovery home/sober living. ***(ability and willingness to work)
• Assistance to find low-income housing (need income)
• Assistance finding employment, GED, and literacy.
• Assistance with ID and AHCCCS application
• Assistance with food/food boxes
